‘I refer to confirmation that Pfizer-BioNTech COVID-19 vaccine batch FN1664 was administered within your Health Board.
To ensure this request remains within cost limits, I am requesting a narrowly defined dataset:
- The number of recorded incidents relating to batch FN1664
- A summary description of each incident (e.g. temperature excursion, storage deviation, handling error)
- Dates of any such incidents
If readily available, please also indicate whether any incidents resulted in:
- Vaccine wastage
- Clinical review or escalation
I am not requesting full reports at this stage.
